GTMRx Releases Patient-Focused CMM Tools

The Get the Medications Right (GTMRx) Institute has released a series of tools designed to provide information about comprehensive medication management (CMM) services to patients and caregivers. The newly released resources were developed by the Best Practices and Innovative Solutions (BPIS) subgroup of the Institute’s Practice and Care Delivery Transformation Workgroup – a diverse, interprofessional group of clinicians charged with developing tools to engage and educate key stakeholders in order to gain support for a standardized definition and process for CMM. ACCP members are prominently represented in the BPIS group. In addition to the Physician-Focused Tools and Medication Optimization Use Cases featured in prior issues of the ACCP Report, this subgroup has developed tools targeting self-insured employers and patient/caregiver stakeholders. Two new tools have been added to those targeting the patient/caregiver audience:

  • A Patient’s Journey Through the Process of Comprehensive Medication Management is an infographic illustrating a patient’s experience with CMM. The graphical representation includes an example of a patient with multiple chronic conditions and medications transitioning between health care settings, highlighting the important role of the clinical pharmacist and CMM in decreasing medication-related problems and restoring the patient’s health. GTMRx will pilot the use of this piece as a marketing tool within a health care institution implementing new CMM services.
  • Comprehensive Medication Management: FAQ for Patients – Informed by members of consumer advocacy groups and presented in question-and-answer format, this document provides information about the value of CMM to patients, caregivers, and the entire health care team.

BPIS is continuing its work on stakeholder toolkits and change packages designed to serve as educational and marketing tools for important stakeholders, with a current focus on tools of relevance to leaders within accountable care organizations.

The BPIS workgroup is one component of GTMRx’s strategy to disseminate the value of CMM to a wide range of stakeholders within health care. Other GTMRx workgroups include the Payment and Policy Solutions, Precision Medicine Enablement via Advanced Diagnostics, Health IT to Support Optimized Medication Use, and Practice and Care Delivery Transformation Evidence Based Resources groups. ACCP members who wish to be considered for participation in future GTMRx projects are encouraged to become signing members and indicate their interest in volunteering for a workgroup.
